Student-Athlete Medical Forms

Required Forms

All student-athletes are required to complete the following forms in annually before participating in Intercollegiate Athletics. All forms will be collected using Medicat. https://carrollu.medicatconnect.com/

1. Sports Health History: Answer all questions regarding your medical history. Complete in Medicat
2. Preparticipation Examination: A physical must be completed every year. This physical needs to be completed within the last six months and must be done by a MD or DO. Upload a copy to Medicat.
3. Emergency Travel  information. You must have valid health insurance that covers athletic injuries. Complete in Medicat
4. Upload a photo copy of the front and back of your insurance card to Medicat.
5. Athletic Risk, Release and Waiver. All student-athletes must complete yearly in Medicat.
6. Concussion Education. CCIW Injury Acknowledgment and Helmet warning in Medicat.
7. Concussion Baseline Test. NCAA legislation mandates that all student-athletes must complete a pre-participation baseline concussion assessment.
8. Sickle Cell Trait Status: All student-athletes must have this information on file. Beginning in summer 2022, all new athletes at Carroll must submit proof of sickle cell results to Medicat. This is completed once per athletic career. See additional information below in Additional forms and links.

The above information is collected using Medicat.

Once you are logged in to Medicat you will have access to:

  • Completing NCAA medical forms
    • Select FORMS in the menu
    • complete all forms and associated uploads listed under NCAA Participation Forms
  • Schedule appointments with Licensed Athletic Trainers
  • Secure communication with Athletic Trainers and Student Health Center
  • Enter immunization records
  • Complete health history forms
